Patients treated in the community are entitled to the same professional quality of care as they would receive when admitted to a hospital. TrakCare Community Health Management module ensures this by providing healthcare professionals with access to all information normally available to them from within their own offices. A patient’s medical history, alerts, referral information and assessments may be viewed; appointments can be booked; and new care plans can be created while still in the patient’s home.
Community management assists in the continuum of care by providing tools to co-ordinate and manage daily planning and activity for staff and resources. Both administrative and clinical details for the client can be recorded.
Community Health Management, like all other TrakCare Modules, is built on the Electronic Patient Record (EPR) and allows the capture, display, and analysis of patient-centric information facilitating better patient care. It is fully integrated with other TrakCare modules and supports all those involved with managing and providing services to the patient beyond the confines of inpatient care.
